Except hw/core/irq.c which implements the forward-declared opaque
qemu_irq structure, hw/adc/zynq-xadc.{c,h} are the only files not
using the typedef. Fix this single exception.

This model implements enough behaviour to do basic functionality tests
such as device initialisation and read out of dummy sample values. The
sample value generation strategy is similar to the STM ADC already in
the tree.

The ADC is part of NPCM7XX Module. Its behavior is controled by the
ADC_CON register. It converts one of the eight analog inputs into a
digital input and stores it in the ADC_DATA register when enabled.

Users can alter input value by using qom-set QMP command.

Some typedefs and macros are defined after the type check macros.
This makes it difficult to automatically replace their
definitions with OBJECT_DECLARE_TYPE.

Patch generated using:

 $ ./scripts/codeconverter/converter.py -i \
   --pattern=QOMStructTypedefSplit $(git grep -l '' -- '*.[ch]')

which will split "typdef struct { ... } TypedefName"
declarations.

Followed by:

 $ ./scripts/codeconverter/converter.py -i --pattern=MoveSymbols \
    $(git grep -l '' -- '*.[ch]')

which will:
- move the typedefs and #defines above the type check macros
- add missing #include "qom/object.h" lines if necessary

<title>include: Make headers more self-contained</title>

Back in 2016, we discussed[1] rules for headers, and these were
generally liked:

1. Have a carefully curated header that's included everywhere first.  We
   got that already thanks to Peter: osdep.h.

2. Headers should normally include everything they need beyond osdep.h.
   If exceptions are needed for some reason, they must be documented in
   the header.  If all that's needed from a header is typedefs, put
   those into qemu/typedefs.h instead of including the header.

3. Cyclic inclusion is forbidden.

This patch gets include/ closer to obeying 2.

It's actually extracted from my "[RFC] Baby steps towards saner
headers" series[2], which demonstrates a possible path towards
checking 2 automatically.  It passes the RFC test there.
