All that is left in mac.h now belongs to the nvram emulation so rename
it accordingly and only include it where it is really used.

Now that the logic related to edge-triggered interrupts is all contained within
the mos6522 device the redundant implementation for the mac99 PMU device can
be removed.

This both allows the wiring to be done as Ben suggested in his original comment in
gpio.c and also enables the OpenPIC object property link to be removed.

The OpenPIC device is located within the macio device on real hardware so make it
a child of the macio-newworld device. This also removes the need for setting and
checking a separate PIC object property link on the macio-newworld device which
currently causes the automated QOM introspection tests to fail.

Some typedefs and macros are defined after the type check macros.
This makes it difficult to automatically replace their
definitions with OBJECT_DECLARE_TYPE.

Patch generated using:

 $ ./scripts/codeconverter/converter.py -i \
   --pattern=QOMStructTypedefSplit $(git grep -l '' -- '*.[ch]')

which will split "typdef struct { ... } TypedefName"
declarations.

Followed by:

 $ ./scripts/codeconverter/converter.py -i --pattern=MoveSymbols \
    $(git grep -l '' -- '*.[ch]')

which will:
- move the typedefs and #defines above the type check macros
- add missing #include "qom/object.h" lines if necessary
