Clean up includes so that osdep.h is included first and headers
which it implies are not included manually.

The variables s390_opformats and s390_num_opformats are unused and
provoke clang warnings:

Also fix disassembly for COMPARE AND BRANCH. The table must be
sorted by primary opcode, and several were out of place.
