Clean up includes so that osdep.h is included first and headers
which it implies are not included manually.

This just catches a couple of stragglers since I posted
the last clean-includes patchset last week.

The test is based on bios-tables-test.c. It creates a file with
the boot sector image and loads it into a guest using PXE and TFTP
functionality.
