Escape sequences received via the serial console can fail since the
cpu_nap() in getchar_timeout() can delay processing for more than the
time it takes for a single character to arrive.

Fix by enabling the UART FIFOs.

serial_console_init() would enable serial console support without
knowing if the serial driver succeeded or not.  As a result, the
serial console would interfere with a normal keyboard on a system
lacking serial support.

<title>[build] Rename gPXE to iPXE</title>

Access to the gpxe.org and etherboot.org domains and associated
resources has been revoked by the registrant of the domain.  Work
around this problem by renaming project from gPXE to iPXE, and
updating URLs to match.

Also update README, LOG and COPYRIGHTS to remove obsolete information.

Add yet another ugly hack to iscsiboot.c, this time to allow the user to
inhibit the shutdown/removal of the iSCSI INT13 device (and the network
devices, since they are required for the iSCSI device to function).

On the plus side, the fact that shutdown() now takes flags to
differentiate between shutdown-for-exit and shutdown-for-boot means that
another ugly hack (to allow returning via the PXE stack on BIOSes that
have broken INT 18 calls) will be easier.

I feel dirty.
