Define a cpu_halt() function which is architecture-specific but
platform-independent, and merge the multiple architecture-specific
implementations of the EFI cpu_nap() function into a single central
efi_cpu_nap() that uses cpu_halt() if applicable.

When ExitBootServices() invokes efi_shutdown_hook(), there may be
nothing to generate an interrupt since the timer is disabled in the
first step of ExitBootServices(). Additionally, for VMs OVMF masks
everything from the PIC (except the timer) by default. This means
that calling cpu_nap() may hang indefinitely. This was seen in
practice in netfront_reset() when running in a VM on XenServer.
Fix this by skipping the halt if an EFI shutdown is in progress.

Entropy gathering via timer ticks is slow under UEFI (of the order of
20-30 seconds on some machines). Use the EFI_RNG_PROTOCOL if
available, to speed up the process of entropy gathering.
Note that some implementations (including EDK2) will fail if we
request fewer than 32 random bytes at a time, and that the RNG
protocol provides no guarantees about the amount of entropy provided
by a call to GetRNG(). We take the (hopefully pessimistic) view that
a 32-byte block returned by GetRNG() will contain at least the 1.3
bits of entropy claimed by min_entropy_per_sample().

Access to the gpxe.org and etherboot.org domains and associated
resources has been revoked by the registrant of the domain. Work
around this problem by renaming project from gPXE to iPXE, and
updating URLs to match.
Also update README, LOG and COPYRIGHTS to remove obsolete information.

Add FILE_LICENCE declarations to almost all files that make up the
various standard builds of gPXE.

Currently the only supported platform for x86_64 is EFI.
Building an EFI64 gPXE requires a version of gcc that supports
__attribute__((ms_abi)). This currently means a development build of
gcc; the feature should be present when gcc 4.4 is released.
In the meantime; you can grab a suitable gcc tree from
git://git.etherboot.org/scm/people/mcb30/gcc/.git
