Mark all files used in a standard build of bin-x86_64-efi/snponly.efi
as permitted for UEFI Secure Boot. These files represent the core
functionality of iPXE that is guaranteed to have been included in
every binary that was previously subject to a security review and
signed by Microsoft. It is therefore legitimate to assume that at
least these files have already been reviewed to the required standard
multiple times.

The PXE TFTP API allows the caller to request a particular TFTP block
size. Since mid-2008, iPXE has appended a "?blksize=xxx" parameter to
the TFTP URI constructed internally; nothing has ever parsed this
parameter. Nobody seems to have cared that this parameter has been
ignored for almost five years.
Fix by using xfer_window(), which provides a fairly natural way to
convey the block size information from the PXE TFTP API to the TFTP
protocol layer.

Access to the gpxe.org and etherboot.org domains and associated
resources has been revoked by the registrant of the domain. Work
around this problem by renaming project from gPXE to iPXE, and
updating URLs to match.
Also update README, LOG and COPYRIGHTS to remove obsolete information.
