Mark all files used in a standard build of bin-x86_64-efi/snponly.efi
as permitted for UEFI Secure Boot. These files represent the core
functionality of iPXE that is guaranteed to have been included in
every binary that was previously subject to a security review and
signed by Microsoft. It is therefore legitimate to assume that at
least these files have already been reviewed to the required standard
multiple times.

Simplify the SMBIOS structure parsing code by assuming that all
structure content is fully accessible via pointer dereferences.
In particular, this allows the convoluted find_smbios_structure() and
read_smbios_structure() to be combined into a single function
smbios_structure() that just returns a direct pointer to the SMBIOS
structure, with smbios_string() similarly now returning a direct
pointer to the relevant string.

Remove the intermediate concept of a user pointer from physical
address conversions, leaving virt_to_phys() and phys_to_virt() as the
directly implemented functions.

The memcpy_user(), memmove_user(), memcmp_user(), memset_user(), and
strlen_user() functions are now just straightforward wrappers around
the corresponding standard library functions.
Remove these redundant wrappers.

Support scanning for the 64-bit SMBIOS3 entry point in addition to the
32-bit SMBIOS2 entry point.
Prefer use of the 32-bit entry point if present, since this is
guaranteed to be within accessible memory.

Extend the syntax for numerical SMBIOS settings from
smbios/<type>.<offset>.<length>
to
smbios/[<instance>.]<type>.<offset>.<length>
Where SMBIOS provides multiple structures with the same <type>, this
extended syntax allows for access to structures other than the first.
If <instance> is omitted then it will default to zero, giving access
to the first instance (and so matching existing behaviour).
The 16-bit SMBIOS handle (which is an alternative way to disambiguate
multiple instances of the same type of structure) can be accessed, if
required, using
smbios/<instance>.<type>.2.2:uint16

Access to the gpxe.org and etherboot.org domains and associated
resources has been revoked by the registrant of the domain. Work
around this problem by renaming project from gPXE to iPXE, and
updating URLs to match.
Also update README, LOG and COPYRIGHTS to remove obsolete information.

Add FILE_LICENCE declarations to almost all files that make up the
various standard builds of gPXE.

EFI provides a copy of the SMBIOS table accessible via the EFI system
table, which we should use instead of manually scanning through the
F000:0000 segment.
