disas does not need to access the CPU env for any reason. Change the
APIs to accept CPU pointers instead. Small change pattern needs to be
applied to all target translate.c. This brings us closer to making
disas.o a common-obj and less architecture specific in general.

Splitting Q and M out of SR, it's possible to optimize div1 by using
TCG code instead of an helper.
At the same time removed the now unused gen_copy_bit_i32 function.

This is improved type checking for the translators -- it's no longer
possible to accidentally swap arguments to the branch functions.
Note that the code generating backends still manipulate labels as int.
With notable exceptions, the scope of the change is just a few lines
for each target, so it's not worth building extra machinery to do this
change in per-target increments.

The TARGET_HAS_ICE #define is intended to indicate whether a target-*
guest CPU implementation supports the breakpoint handling. However,
all our guest CPUs have that support (the only two which do not
define TARGET_HAS_ICE are unicore32 and openrisc, and in both those
cases the bp support is present and the lack of the #define is just
a bug). So remove the #define entirely: all new guest CPU support
should include breakpoint handling as part of the basic implementation.

This will collect all load and store helpers soon. For now
it is just a replacement for softmmu_exec.h, which this patch
stops including directly, but we also include it where this will
be necessary in order to simplify the next patch.

softmmu_template.h twice for each size, io_readX must be elided the
second time through.

Rather than include helper.h with N values of GEN_HELPER, include a
secondary file that sets up the macros to include helper.h. This
minimizes the files that must be rebuilt when changing the macros
for file N.

Most targets were using offsetof(CPUFooState, breakpoints) to determine
how much of CPUFooState to clear on reset. Use the next field after
CPU_COMMON instead, if any, or sizeof(CPUFooState) otherwise.

Everything before CPU_COMMON in the structure is cleared as part of a
CPU reset. This included the features flag, which indicates whether SH4A
instructions are supported or not. As a result, a CPU reset downgraded
the CPU from an SH4A to an SH4.

Commit c643bed99 moved qemu_init_vcpu() calls to common CPUState code.
This causes x86 cpu-add to fail with "KVM: setting VAPIC address failed".
The reason for the failure is that CPUClass::kvm_fd is not yet
initialized in the following call graph:
->x86_cpu_realizefn
->x86_cpu_apic_realize
->qdev_init
->device_set_realized
->device_reset (hotplugged == 1)
->apic_reset_common
->vapic_base_update
->kvm_apic_vapic_base_update
This causes attempted KVM vCPU ioctls to fail.
By contrast, in the non-hotplug case the APIC is reset much later, when
the vCPU is already initialized.
As a quick and safe solution, move the qemu_init_vcpu() call back into
the targets' realize functions.

This avoids polluting the global namespace with a non-prefixed macro and
makes it obvious in the call sites that we return.
Semi-automatic conversion using, e.g.,
sed -i 's/GET_REGL(/return gdb_get_regl(mem_buf, /g' target-*/gdbstub.c
followed by manual tweaking for sparc's GET_REGA() and Coding Style.

CPUState::gdb_num_regs replaces num_g_regs.
CPUClass::gdb_num_core_regs replaces NUM_CORE_REGS.
Allows building gdb_register_coprocessor() for xtensa, too.
As a side effect this should fix coprocessor register numbering for SMP.

Change breakpoint_invalidate() argument to CPUState alongside.
Since all targets now assign a softmmu-only field, we can drop helpers
cpu_class_set_{do_unassigned_access,vmsd}() and device_class_set_vmsd().

x86 was using additional CPU_DUMP_* flags, so make that configurable in
CPUClass::reset_dump_flags.
This adds reset logging for alpha, unicore32 and xtensa.

Since commit 878096eeb278a8ac1ccd6667af73e026f29b4cf5 (cpu: Turn
cpu_dump_{state,statistics}() into CPUState hooks) CPUArchState is no
longer needed.
Add documentation and make the functions available through qemu/log.h
outside NEED_CPU_H to allow use in qom/cpu.c. Moving them to qom/cpu.h
was not yet possible due to convoluted include paths, so that some
devices grow an implicit and unneeded dependency on qom/cpu.h for now.
