In the majority of pages, pathnames are formatted as Italic,
which is the norm. However, there are several cases where they
are formatted as bold. This patch fixes a number of those
exceptions.

of formatting page cross references in bold. Fix the
few exceptions that use italic.

Protect a full stop (.), that begins or ends a string, with \&

The old version in some cases (but not always) returns -1 (255) on
error. It seems better to cleanup the code and don't return internal
return codes by exit().

Use dates without the day, use the full month name, put "util-linux" in
the lower left corner, and "User Commands" or "System Administration"
at the top center.
Also improve here and there the one-line program description.

$ chsh
chsh: can only change local entries; use ypchsh instead.
The 'ypchsh' is an external util that does not have to be installed.
